Renewal of my Son Passport

Hello Guys, I am a Portuguese passport holder leaving in Krumbach Bavaria, and my wife a Romanian..our kids also have a Romanian passports. I need to renew the passport of my Son for which I need to make a declaration.
(Declaratia de consimt膬m膬nt p膬rinte) Any Romanian National who has done this before or if somebody can give me some info will be very much appreciated.
We did contact the embassy in Munich and we filled the online form but need this document to attach for the file. Thank youin advance.

You can find the requirements for the document here.聽 It's really only a notarized declaration, not a pre-made form.
