good question.  I would suggest going to your nearest EBAIS clinic (Caja) and ask the reception person... who may know nothing... so ask the nurse.  The nurse may know nothing... so ask the doctor or even one of the 'smarter' persons working in the pharmacy of the clinic.  Somebody will have some sort of clue as to how/where to get that exam.   Depends on where you live; which you didn't tell us.

Good luck.

You might also pose your questions to a local motorcycle dealer or repair shop.

I'm not clear about what more involved an exam than the usual dictamen. Are you looking for an orthopedist's evaluation?

i am planning my move which could be six months into the future.  we will be coming down for a few weeks then determine where. but access to medical care is on the top of the list.  i don't want to drive 3 hours for a doctor's appointment.  it could change after we have been there but our first thoughts are Lake Arnell Area or about 10 miles off the Pacific Coast  We enjoy fishing and want to do a lot.  It is her decision where, but medical  access will made a big difference where.  San Jose area is off the list.  Too much crime in some parts.  That is part of why I am leaving the US.  i think the us will go broke in 7 to 9 years.  The government just keeps borrowing money.  this year out of every dollar income taxes it cost 40% just to pay the interest.  next year even with a 9% reduction in spending they are still borrowing 6% to keep the government going.  Venezuela did the same thing, one day law enforcement next day no government crime went thru the roof.  Good people had no choice but steal and it was very dangers for about six months.  That is one horse I don't want to ride.  Costa Rica has crime but still lower than the US today.  i am leaving and coming to Costa Rica.  thank you for your post
